Information Technology Implementation Plan Workgroup
The Information Technology Implementation Plan Workgroup is one of three workgroups supporting the planning and development of the IBH model. The workgroups will meet virtually between larger convening structure meetings and bring together stakeholders, including providers, community agencies, IPAs, health plans, and technology partners.
The Information Technology Implementation Plan Workgroup will focus on identifying the health information technology infrastructure needed to support the implementation of the IBH model, including care coordination, data sharing, information exchange, and reporting.
The workgroup will develop draft plans and recommendations for review by the full convening structure and to inform New York State’s design and implementation of the IBH model.
