Care Delivery Framework Workgroup
The Care Delivery Framework Workgroup is one of three workgroups supporting the planning and development of the IBH model. The workgroups will meet virtually between larger convening structure meetings and bring together stakeholders, including providers, community agencies, IPAs, health plans, and technology partners.
The Care Delivery Framework Workgroup will focus on developing an integrated care delivery framework that connects behavioral health, physical health, and upstream drivers of health. The workgroup will explore approaches to support coordinated, integrated, and person-centered care within the IBH model.
The workgroup will develop draft plans and recommendations for review by the full convening structure and to inform New York State’s design and implementation of the IBH model.
