Overview
The Innovation in Behavioral Health (IBH) model aims to provide evidence based integrated physical and behavioral health care for people with moderate to severe behavioral health conditions.
New York State, through the Department of Health, was chosen by the Center for Medicare and Medicaid Innovation to participate in an 8-year project focused on creating an innovative comprehensive integrated behavioral health, physical health, and upstream drivers of health model. The goal of this project is to engage Western NY regional partners to develop and/or adopt data-driven clinical best practices while working together on the creation of sustainable advanced payment models.
